Can I see an example/sample of an alarm certificate?

I ordered a system on 12/17/2011 and must present an alarm certificate to my insurance company (International Assurance) prior to 01/15/2012 in order to receive a discount credit of $357.31. Can someone post a sample alarm certificate so that I can check to make sure that my insurance company is okay with it?
I had an ADT system previously and am happy to be done with them! Worst CS ever!!!

Thanks for posting! Although I cannot provide an example of a certificate, most insurance companies accept it as proper documentation of a monitored system. Once you activate your service online, the certificate is immediately available from your customer account! It states your name, the service address, and that you are monitored. It is also signed and dated, ready to go! All you need to do is print it out!
